چکیده

BACKGROUND AND PURPOSE Arterial recanalization and subsequent reperfusion have extensively demonstrated their ability to restore the brain function when performed shortly after acute ischemic stroke. However, arterial recanalization does not necessarily lead to brain tissue reperfusion. SUMMARY OF REPORT This review provides an update of current approaches to improve the efficacy profile of brain tissue reperfusion within and beyond the therapeutic window, including the use of novel thrombolytic agents, bridging intravenous and intra-arterial therapies, and mechanical clot retrieval or aspiration. CONCLUSIONS There are still several challenges in the near future of reperfusion therapy for acute ischemic stroke, such as improving the ultra-early access to treatment within the "golden hour," extending the therapeutic window beyond the current 4.5-hour time window, and developing novel thrombolitics or combined approaches to improve treatment efficacy.

Fibrinolytic Therapy in Acute Stroke

Acute ischemic stroke is a major cause of morbidity and mortality in Europe, North America, and Asia. Its treatment has completely changed over the past decade with different interventional approaches, such as intravenous trials, intra-arterial trials, combined intravenous/intra-arterial trials, and newer devices to mechanically remove the clot from intracranial arteries. Intravenous thrombolys...

The eJournal of the European Society of Minimally Invasive Neurological Therapy Thrombectomy for Acute Ischemic Stroke Treatment: A Review

Mechanical treatment approaches for acute ischemic stroke treatment aim for fast and efficient reperfusion with short procedure times and high recanalisation rates, thus extending the treatment window. We review the current literature on mechanical thrombectomy for the treatment of acute ischemic stroke.

Extending reperfusion therapy for acute ischemic stroke: emerging pharmacological, mechanical, and imaging strategies.

BACKGROUND AND PURPOSE Reperfusion is the most beneficial of all therapeutic strategies for acute ischemic stroke. However, the standard cerebral reperfusion treatment of the first decade of the reperfusion era, noncontrast computed tomography (CT)-guided, < or =3 hours, intravenous tissue plasminogen activator, has many limitations. This review surveys emerging strategies that have the potenti...

O10: Thrombo-Inflammation in Acute Ischemic Stroke

Ischemic stroke has been classified as a merely thrombotic disease, so the main goal of its treatment is the recanalization of the occluded vasculature. However, despite fast restoration of blood circulation, progressive stroke still develops in many patients, which has led to the concept of reperfusion injury. The underlying mechanism is only partly known. Though, it is accepted now, tha...

Emerging therapies for acute ischemic stroke: new therapies on trial.

The development of additional effective therapies for acute ischemic stroke remains a challenging but critical endeavor. Intravenous recombinant tissue plasminogen activator (rtPA) initiated within 3 hours of stroke onset remains the only approved and validated therapy for acute ischemic stroke, and regulatory approval has expanded recently. Many other therapies have been evaluated, and these t...
